After-Sale Service

In case of after-sale service claim, Customers should fill out a Service Claim Form* with detailed information including but not limited to:
a.Model Name;
b.Serial Number (S/N);
c.Specific trouble condition.
Biolilght Meditech USA should have no obligations to accept any service claims without the receipt of fulfilled Service Claim Form.
After filling out the Service Claim Form, please return it by email to service@biolightmeditechusa.com .
Once a fully filled Service Claim Form is received, Biolight Meditech USA will notice the Customer and offer a solution within three working days

The service claim will be followed up on the following two occasions:
1) Within Warranty:
There are two options given:

a) After receiving reply from Biolight Meditech USA, please follow up our after - sale service engineers¡¯ instruction.
If sending the defective part back to Biolight Meditech USA is requested, please inform us the shipment tracking number.
Biolgiht Meditech USA will dispatch replacement part in exchange of the defected part to assigned address.

b) Please signs a Declaration Form and sends the copy to Biolight Meditech USA by either email or fax.
This Declaration Form is legally certificated to guarantee that the Customer will return the defective part to Biolight Meditech USA on time.
Biolight Meditech USA will, at this stage, dispatch the replacing part to assigned address.

2)Out of Warranty:
a) After receiving reply from Biolight Meditech USA, and sending the defective part to Biolight Meditech USA is requested, please inform us the shipment tracking number soon afterwards.
Once receiving it, we will analyze the degree of trouble condition and offer a quotation to The Customers of either repairing defective part or making replacement.
Provided the maintenance fee is paid, Biolight Meditech USA will dispatch replacement part to assigned address.

Packing Suggestions:
In case of returning goods for service purpose, please follow these recommended packing instructions:
1) Please disassemble the parts with anti-static facility, do not touch the parts with naked hand.
2) Remove all hoses, cables, sensors, transducers and power cords before packing, unless you suspect they are associated with the problem.
3) Wherever possible use the original shipping carton or packaging affording an equal degree of protection.
4) Put the Service Claim Form Number on the parcel.
5) It is recommended that all returned goods are insured. Claims for loss or damage of the product must be initiated by the sender.

Note: Biolight Meditech USA should have no obligations to accept any unauthorized returned goods from either the Customer or end-user of the Customer.
The Customer takes the responsibility of accounted fee on such occasions.

Freight Policy
1)Within Warranty:
The Customer is responsible for freight & insurance charges of the shipment to Biolight Meditech USA (Customs charge included if needed).
Biolight Meditech USA will undertake the freight & insurance charges of the shipment to the Customer.

2)Out of Warranty:
The Customer is responsible for freight & insurance of the shipment both to and from Biolight Meditech USA.
